Ready to go to camp?

We will forward the names of the children in this form to the appropriate camp(s).  Camps should be reaching out to campers to register them for camp.

NOTE: Some camps do not have the staff or capacity to do so.  You will be responsible for making sure the child is registered for camp; either online / phone / form, etc.

Camps have been instructed to ask parents/guardians to contribute to camper fees when possible.  This will help us impact as many lives as possible and allow for more children to attend camp.  Every year we have more registrations than we have dollars, so lets try and make it go the farthest.

If there are multiple children that wish to register, please fill out the form for each child.

The process is as follows:

1. Fill out the form
2. We will send child's infomation to the appropriate camp
3. Register your child for the appropriate camp - independant of this form
4. Child goes to camp - the camp sends the invoice of remaining amount to The 1000 Mile Challenge
5. 1000 Mile Challenge reconciles invoices with our registration list and pays invoice to camp.
